• Post-surgical recovery (typically 8-12 weeks post-op)
• Chronic or complex medical conditions

The goal of the multidisciplinary assessment is to:

• Includes: Physical and functional evaluations, medical and psychosocial screening, and a unified report from all involved disciplines
